Better Results Proven with In-House Therapy
The Asísta family of Health Centers takes extraordinary steps to achieve successful results for our customers, including a bold move to bring professional occupational, physical and speech therapists inside our Health Centers as employees on a full-time basis.

This unique level of daily, on-site involvement and service dramatically increases the opportunities for therapy and allows the care for each client to be therapy-driven, achieving higher levels of mobility, regained health and independence much more quickly.

Our Results show greater numbers of our clients are going back home to remain independent than ever before.

Evaluation and Care Process
Frequently, if a client is coming from the hospital following a health crisis like a fall, stroke or heart attack, the client may be bewildered and the family desperately in need of help. We ease the anxiety and calm the situation. Upon admission, each client is evaluated by both occupational and physical therapists and a Plan of Care is established.

Following admission, the client is evaluated for strength, endurance, and whether adaptive equipment is needed. For speech therapy clients, a re-learning and retraining program is established. During this process a family meeting will be scheduled with a member of the Rehab team and social services. The Care Plan Team meets twice a week with the physician. The Team also meets with the family to learn about any specifics related to the clients’ health.

Medicare B clients needing longer-term care or are having a decline in function are screened and evaluated by occupational, physical and speech therapists.

Unique Formal Integration between Doctors and Therapy
More than simply executing written doctors’ orders, at our Health Centers, geriatric physicians are a part of the therapy process. The Care Plan Team actually meets with and integrates the physician into the therapy process. Family members are included at any time. This very integrated program makes it easy to identify any special client needs in one-on-one communication. Good communication allows for client and family to understand why we are doing what we are doing and what the process will be.

Unique Home Safety Assessment
During the process of rehabilitative care, a family member is asked to come pick up the client and take them back home; they are followed by one of our therapists who conducts a Home Safety Assessment. This evaluation determines what is needed to allow the client to function and manage being at home again. Adjustments are then made in the care.

Unique Restorative Department Follows After Therapy is Completed
For long-term clients after rounds of therapy are completed, our Health Centers offer a unique service for up to twelve weeks which may continue to improve results. Restorative Care is provided by a certified nursing assistant with specialized training from the therapy department. If a client is interested in continuing to participate, an Exercise Program is offered one time per day, five days per week, and once on the weekend at a number of Health Centers within the Asísta family.

Fall Prevention
The Fall Prevention Program is a specially designed Rehab and Nursing program that is centered around the prevention of falls. When a resident falls, or has potential to fall, they are put into the Fall Prevention Program. The Fall Committee meets two times a month and reviews each resident's chart and possible Fall/Incident report to better problem-solve and find safer solutions, environmental modifications, therapeutic interventions that will hopefully prevent future accidents. Even though the meetings are two times a month, this is an ongoing everyday process.

Walking Program
Following therapy and the Restorative program, clients may be put into a walking program with a Restorative Aid five times per week to regain mobility.

Therapeutic Dining
For any client that requires assistance to be able to feed themselves and/or to swallow, a small environment run by Restorative Aids and Nurses has been developed. The program takes place in the smaller dining room, near the large dining room.
